That’s because the COBOL OGs are retired/ing and the industry has been training young people telling them “yeah, sorry, this is all we can pay you”. Here in Europe, they’ll take unemployed people from a different industry, put them on a training course, and bang! you’ve got a grateful new dev who doesn’t know how much they are worth.
You just gotta keep spreading the message. I keep happily sharing my salary, especially with younger, less experienced devs, so we can all win better.

Admittedly, just sharing your own may make some people feel pressured to share theirs out of reciprocity, but just sharing your own salary generates nowhere near the same amount of pressure as outright telling someone “share your salary or you’re a bad person on the side of The Man!”

I hope the amount of people sharing their salary increases and talking about it becomes normalized.

Yeah, inferior turbinate reduction is the next small step for this. Often if it’s just alternating nasal obstruction that’s good enough. Oftentimes there’s another component of nasal valve collapse or septal deviation. Personally, in my population, I end up doing septorhinoplasty (nose job) way more often than other smaller nasal surgery.

You don’t want them to actually remove the turbinates, however. We generally just shrink them down – removing them makes the nasal air less turbulent, and difficult to sense airflow. TL;DR it make look like you can drive a semi truck through the nose, but people will feel like they cannot breathe at all. People have killed themselves over this.

It can happen, but the way most ENTs train these days, unlikely. I’ve seen it twice that I recall off the top of my head, but very rare these days.

Most ENTs, including myself, are overly cautious. You’re at a higher risk for symptom recurrence because of under resection.

That being said, I wouldn’t let an oral surgeon or general plastic surgeon touch my family member’s nose (unless they had a very very good reputation). Nothing wrong with their work, I’m just not sure they had the same training and respect for the nose.

This applies to nasal decongestants (NOT nasal steroids). Nasal decongestants (such as oxymetazoline AKA afrin, or phenylephrine based medications) are vasoconstrictors. They work very well and work very quickly as the vasoconstriction (constricting the blood vessels) which shrinks the inferior turbinates (and any other edematous tissue).

The body responds to chronic vasoconstriction by making more blood vessels. When the nasal tissues have more blood vessels (and I presume are more dense with vessels) it’s harder for the decongestant to work. This is called rebound congestion — conversely, the patients in this scenario will feel they need to use more decongestant since it previously worked so well, but it no longer does. This cycle can be challenging to treat.

For this reason most ENTs, including myself, typically recommend against afrin use for more than 3 consecutive days. I’ve seen who go as long as five, but I’m cautious and would not recommend more than 3 days.

It’s a bit funny, because if you come into my clinic and get an endosocpic exam of the nose and/or throat (i.e. probably around 50%, often more, of my patients on any given day), I will spray afrin and lidocaine into the nose before my examination. The other main thing I use it for is nosebleeds. It’s okay to use it for 3 days during an acute exacerbation of sinusitis, but I don’t really think it’s necessary.

Edit: I forgot to mention nasal steroids. As I said, the above response doesn’t apply to them. We don’t include nasal steroids in this because they have a very slow effect and don’t have the effect of rebound congestion. With few exceptions doing 2 sprays each nostril daily for a very long is fine for almost everybody, and usually helpful. When I prescribe them I recommend patients use them for at least 4 weeks. Once in awhile there are patient that I would be more cautious with prescribing nasal steroids, such as those with a septal perforation, or frequent nose bleeds. Usually it’s a non issue. Tip: when spraying them don’t spray straight back – use your opposite hand and spray towards the eye (i.e. spray with right hand into left nostril, aiming towards left eye).

I wish it was that easy. Flonase did nothing for me other than cause my nose to bleed. Azelastine helped some though and that makes sense for me, my congestion is likely caused by dust mite allergy.

Affrin is amazing for what it is. A few weeks ago I had a sinus headache so bad I threw up. Affrin cleared it up in 30 seconds, and I was able to maintain it without affrin after that.

Nosebleeds can happen and certainly do for some. Nasal hydration helps (for instance, ayr gel in combination with saline spray or irrigations). Ultimately, a good portion of patients that don’t tolerate or fail nasal steroids get surgery.

Azelaetine is fantastic - there’s a lot of patients I prescribe it in conjunction with Flonase. Allergic rhinitis or even just excessive secretions is common in patients with inferior turbinate hypertroph/nasal obstruction, and both meds have a function. They sell it as a combination, actually, but often insurance doesn’t cover the combo.

Try a good deli-style American. So much better than Kraft Singles. Much lower milk content, so it’s more solid. Not individually wrapped, but individually sliced. Higher cheddar and Colby content, so its flavor is better. Still melts beautifully. Best burger cheese out there, IMO.

American cheese is processed. Depending on the amount of milk added during that processing, you get a product with a more or less firm consistency.

Singles are liquid when they’re poured into the wrap, and then they solidify. In this they end up being far less like traditional cheese.

A grilled cheese consists of only these following items. Cheese. Bread with spread (usually butter). This entire subreddit consist of “melts”. Almost every “grilled cheese” sandwich i see on here has other items added to it. The fact that this subreddit is called “grilledcheese” is nothing short of utter blasphemy. Let me start out by saying I have nothing against melts, I just hate their association with sandwiches that are not grilled cheeses. Adding cheese to your tuna sandwich? It’s called a Tuna melt. Totally different. Want to add bacon and some pretentious bread crumbs with spinach? I don’t know what the hell you’d call that but it’s not a grilled cheese. I would be more than willing to wager I’ve eaten more grilled cheeses in my 21 years than any of you had in your entire lives. I have one almost everyday and sometimes more than just one sandwich. Want to personalize your grilled cheese? Use a mix of different cheeses or use sourdough or french bread. But if you want to add some pulled pork and take a picture of it, make your own subreddit entitled “melts” because that is not a fucking grilled cheese. I’m not a religious man nor am I anything close to a culinary expert. But as a bland white mid-western male I am honestly the most passionate person when it comes to grilled cheese and mac & cheese. All of you foodies stay the hell away from our grilled cheeses and stop associating your sandwich melts with them. Yet again, it is utter blasphemy and it rocks me to the core of my pale being. Shit, I stopped lurking after 3 years and made this account for the sole purpose of posting this. I’ve seen post after post of peoples “grilled cheeses” all over reddit and it’s been driving me insane. The moment i saw this subreddit this morning I finally snapped. It’s my favourite for burgers. It lasts way longer without going bad (I find real cheese slices will go moldy before I can use them all), tastes better to me, and is meltier. The meltiness is really nice!

Kraft does sell non-singles sliced american cheese. Nothing between the slices and it doesn’t stick together. Also doesn’t have that plasticy taste.

Water rights are the opposite of late stage capitalism. It’s silly to enforce when we’re talking about a residential rain barrel, but when we’re talking on much larger scales is critical. When creeks are drying up because landowners are building catchment ponds, water rights start to look pretty good.

It’s because Colorado water law is based on ‘prior appropriations’.

Colorado was settled around mining and ranching, both of which can be water-intensive. It’s also a fairly dry place. Water rights have been serious business for a long time.

So the rule was that the first person there had the right to start using river water for their mine. Then, if a second person starts a mine upstream, they had the right to use river water only inasmuch as it didn’t impact the prior downstream mine. If there was a drought, the upstream mine had to use less water so the earlier mine wasn’t impacted. Rain barrels were prohibited because that water “belonged” to some downstream rights holder, just as using the water from a stream might be prohibited because it belongs to a downstream rights holder.

This isn’t really late-stage capitalism. The law in Colorado goes back to some court cases in the 1870s and 1880s.

Sometimes you have to think about broad impact when developing policy. Sure, laws against rain collection seem draconian on the individual scale, but if a large percentage of the population collected rainwater, reservoirs and water tables can be seriously affected. Not saying this specific Israeli action is justified, but there are valid limitations on water collection put in place to ensure everyone has access.

It would be substantially worse if there were no such limitations in place, and whoever owned the land that drained into communal reservoirs could privately control the water supply of a region.
