TITLE:
Demographic predictors of emergency service utilization patterns in youth at risk of suicide

<h2>Abstract</h2>
<h2>Objective</h2>
<p>To explore demographic predictors of Emergency Department (ED) utilization among youth with a history of suicidality (i.e., ideation or behaviors).</p>
<h2>Methods</h2>
<p>Electronic health records were extracted from 2017 to 2021 for 3094 8–22 year-old patients with a history of suicidality at an urban academic medical center ED in the Mid-Atlantic. Logistic regression analyses were used to assess for demographic predictors of ED utilization frequency, timing of subsequent visits, and reasons for subsequent visits over a 24-month follow-up period.</p>
<h2>Results</h2>
<p>Black race (OR = 1.45, 95% CI = 1.11–1.92), Female sex (OR = 1.59, 95% CI = 1.26–2.03), and having Medicaid insurance (OR = 1.71, 95% CI = 1.37–2.14) were associated with increased utilization, while being under 18 was associated with lower utilization (&lt;12: OR = 0.38, 95% CI = 0.26–0.56; 12–18: OR = 0.47, 95% CI = 0.35–0.63). These demographics were also associated with ED readmission within 90 days, while being under 18 was associated with a lower odds of readmission.</p>
<h2>Conclusions</h2>
<p>Among patients with a history of suicidality, those who identify as Black, young adults, patients with Medicaid, and female patients were more likely to be frequent utilizers of the ED within the 2 years following their initial visit. This pattern may suggest inadequate health care access for these groups, and a need to develop better care coordination with an intersectional focus to facilitate utilization of other health services.</p>

High Prevalence of Rectal Chlamydia and Gonorrhea Among Men Who Have Sex With Men Who Do Not Engage in Receptive Anal Sex

<p>In the United States, annual screening for rectal gonorrhea and chlamydia is only recommended for men who report receptive anal sex (RAS), but other behaviors (e.g., rimming) may lead to rectal Chlamydia trachomatis and Neisseria gonorrhoeae acquisition.</p>
<p>Methods </p>
<p>We enrolled individuals assigned male sex at birth who reported sex with men and denied RAS in the past 2 years or reported RAS 1 to 2 years ago but were tested and treated since last RAS. Participants enrolled in-person at the Sexual Health Clinic in Seattle, Washington (December 2019–July 2022), or online (July 2021–March 2022). Participants completed a survey that asked about 13 non-RAS behaviors and self-collected a rectal swab for gonorrhea/chlamydia nucleic acid amplification testing. We used log binomial regression to estimate the prevalence of rectal gonorrhea/chlamydia (adjusted prevalence ratio [aPR]) by behavior, adjusting for all other behaviors.</p>
<p>Results </p>
<p>We enrolled 292 participants (247 in-person and 45 online); 277 (95%) had nucleic acid amplification testing results. Rectal gonorrhea/chlamydia test positivity was 14.1% overall: 10.5% for rectal chlamydia and 4.3% for rectal gonorrhea. Most participants (70%) reported ≥1 behavior that involved direct contact with their anus. We observed a higher risk of rectal chlamydia for those who did versus did not report perianal play at 12 months (aPR, 2.39; 95% confidence interval, 1.10–5.22) and 2 months (aPR, 2.21; 95% confidence interval, 1.02–4.79). This was the only behavior significantly associated with testing positive.</p>
<p>Conclusions </p>
<p>Rectal C. trachomatis and N. gonorrhoeae prevalence was high among men who deny RAS, suggesting other possible routes of acquisition. Rectal screening for those who deny RAS should be made with careful consideration of individual- and population-level effects.</p>

Descriptive Analysis of Documentation Time for the National Developmental-Behavioral Pediatric Physician Workforce Using a Commercial Electronic Health Record System

<p>Objective: </p>
<p>The national developmental-behavioral pediatric (DBP) workforce struggles to meet current service demands because of several factors. Lengthy and inefficient documentation processes are likely to contribute to service demand challenges, but DBP documentation patterns have not been sufficiently studied. Identifying clinical practice patterns may inform strategies to address documentation burden in DBP practice.</p>
<p>Methods: </p>
<p>Nearly 500 DBP physicians in the United States use a single commercial electronic health record (EHR) system (EpicCare Ambulatory, Epic Systems Corporation, Verona WI). We evaluated descriptive statistics using the US Epic DBP provider data set. We then compared DBP documentation metrics against those of pediatric primary care and selected pediatric subspecialty providers who provide similar types of care. One-way analyses of variance (ANOVAs) were conducted to determine whether outcomes differed among provider specialties.</p>
<p>Results: </p>
<p>We identified 4 groups for analysis from November 2019 through February 2020: DBP (n = 483), primary care (n = 76,423), pediatric psychiatry (n = 783), and child neurology (n = 8589). Post hoc pairwise comparisons revealed statistically significant differences between multiple outcome-specialty combinations. Time in notes per appointment and progress note length demonstrated the strongest evidence of an increased burden on DBP providers compared with other similar provider groups.</p>
<p>Conclusion: </p>
<p>DBP providers spend a significant amount of time documenting progress notes both during and outside of normal clinic hours. This preliminary analysis highlights the utility of using EHR user activity data to quantitatively measure documentation burden.</p>

<p class="a-plus-plus">Because of COVID-19 pandemic, families across the world are experiencing new stressors that threaten their health, and economic well-being. Such a stress may jeopardize parents-children relationship. We aim to investigate the magnitude of child physical maltreatment (CPM) by parents in Egypt during the COVID-19 pandemic, to relate it to parents’ stress, and to identify other potential risk factors.</p>

<h3 class="a-plus-plus">Methods</h3>
<p class="a-plus-plus">This cross-sectional study assessed parent-reported CPM and their personal experience of stress, depression, and anxiety among a sample of Egyptian parents using an electronic survey. It included sociodemographic data, Depression, Anxiety and Stress Scale (DASS-21), and the Child physical maltreatment scale (CPMS). We also briefly assessed COVID-19 -related data.</p>

<span class="a-plus-plus abstract-section id-a-sec3"></p>
<h3 class="a-plus-plus">Results</h3>
<p class="a-plus-plus">Out of 404 respondents, (62.9%) and (32.9%) reported performing minor and severe CPM toward their children during the past 3 months, respectively. The age of youngest child, and anxiety score were significantly correlated with both minor and severe forms of CPM. While number of children, and online education system ratings were only significantly correlated with severe CPM. Parental definition of CPM was significantly correlated to minor CPM, but not to severe CPM.</p>

<h3 class="a-plus-plus">Conclusions</h3>
<p class="a-plus-plus">CPM by parents is not uncommon in Egypt, especially during the COVID-19 pandemic. These findings highlight the importance of regular support and intervention that help parents learn parenting skills and the use of non-violent child disciplining methods.</p>

Dyadic process and dynamics of caregiving and receiving in suicide recovery among Filipino college‐aged students and their families

<h2>Abstract</h2>
<p>With the increasing incidence of suicidality among the youth, identifying effective care and support that family members provide is essential. Despite numerous studies on the link between suicide mitigation and caregiving, the dyadic process and dynamics of family members supporting youth at risk have been scarcely explored. This study utilizes grounded theory to explore the actions, interactions, and processes involved in caregiving and receiving between five pairs of Filipino family caregivers and college-aged care receivers who recovered from suicidality. The study gave rise to a seven-phase model that characterizes the dynamic dyadic interactions between the family caregiver and the youth care receiver. This is summarized in the acronym C<sup>2</sup>A<sup>2</sup>R<sup>2</sup>E, which means calling-on, contemplating, accepting, allowing, responding, reciprocating, and empowering. This model highlights the processes and dynamics of care within families, which may be useful in paving the way for families and mental health professionals to foster more effective support in mitigating suicidality among youth at risk.</p>

<p><imgsrc="" border="0" align="left" alt="image">Purpose of review </p>
<p>This review provides an overview of recent developments in understanding polysubstance use patterns across the lifespan, and advances made in the prevention and treatment of harm arising from polysubstance use.</p>
<p>Recent findings </p>
<p>A comprehensive understanding of the patterns of polysubstance use is hampered by heterogeneity across study methods and types of drugs measured. Use of statistical techniques such as latent class analysis has aided in overcoming this limitation, identifying common patterns or classes of polysubstance use. These typically include, with decreasing prevalence, (1) Alcohol use only; (2) Alcohol and Tobacco; (3) Alcohol, Tobacco, and Cannabis; and finally (4) a low prevalence, Extended Range cluster that includes other illicit drugs, New Psychoactive Substances (NPS), and nonmedical prescription medications.</p>
<p>Summary </p>
<p>Across studies, there are commonalities present in clusters of substances used. Future work that integrates novel measures of polysubstance use and leverages advances in drug monitoring, statistical analysis and neuroimaging will improve our understanding of how and why drugs are combined, and more rapidly identify emerging trends in multiple substance use. Polysubstance use is prevalent but there is a paucity of research exploring effective treatments and interventions.</p>

Suicidality and Implicit Dependency in Child and Adolescent Inpatients With Histories of Trauma: Moderating Effect of Gender

<p><imgsrc="" border="0" align="left" alt="image">There is lack of empirical findings on a direct link between suicidality and dependency in youth. This is particularly relevant for children and adolescents with a trauma history, since traumatization is a well-established risk factor for suicidality in this population. Research on dependency predominantly uses self-report assessments, which may be susceptible to biases. In this study, performance-based interpersonal dependency scores in inpatient children and adolescents with trauma history were compared with patients&; suicidal behavior (suicidal ideation and suicidal attempts) as derived from chart records. The results showed a gender effect. High dependency scores were associated with higher suicidal ideation for girls and with lower suicidal attempts for boys. These findings demonstrate that a relationship between dependency and suicidality for hospitalized traumatized youth is impacted by gender.</p>
